New Patient Center

STEP 1: Complimentary Neurological Chiropractic and/or Low Level Laser Consultation:

There is no charge for an initial new patient consultation. This is your opportunity to meet Dr. Burdorf at his Scottsdale office to discuss your case and determine whether or not it’s a good fit to work together. Dr. Burdorf is affiliated with the picture archived computer systems, (PACS) of Scottsdale Medical Imaging, (SMIL), Simon Med and Insight. Quick access to your prior diagnostic studies and the ability to refer you for new imaging, if necessary, saves our patients money and time. Dr. Burdorf will determine if we will accept you as a patient.

Science-Based Nutrition Consultation:

Are you interested if you are taking the right supplements, and are they effective? Our office uses a Science-based approach by recommending supplements based on your bloodwork. There is no charge for the initial 15 min consultation. Dr. Burdorf will determine which tests will be needed to proceed during this consultation.

STEP 2: New Patient Appointment:

Once you have completed the 15 min complimentary consultation, and all parties have decided to move forward with a treatment plan, Dr. Burdorf will complete a comprehensive review of your health history and perform an extensive physical and neurological examination.  If necessary, Dr. Burdorf may ask for x-rays, and will refer you to a local imaging facility.  Dr. Burdorf will recommend a treatment plan for care at our office. This appointment with the doctor will last approximately an hour. For your convenience and to save time during your first visit, download and fill out the Case History, Consent For Treatment and Functional Rating Index forms below.

STEP 3: Execution Of Ongoing Treatment Plan:

After you have completed the 15 min complimentary consultation and your new patient appointment, Dr. Burdorf will initiate the treatment plan that is specific to your case. Your treatment plan will be dependent on your presenting complaint/request.
